Waiver Forms

ALL passengers boarding Lady A for any Day Charter Tour are required to fill out a BOAT PASSENGER WAIVER. For your convenience, we have added the Adult & Minor forms to the site in order that you may download, print, fill out and submit to Captain Harry upon embarking Lady A. Please note that there are no exceptions to the requirement for ALL passengers regardless of age to submit a BOAT PASSENGER WAIVER prior to initiating a Day Charter on Lady A. We thank you in advance for your cooperation and we look forward to serving you aboard Lady A.
